Our team of doctors follow you through your pregnancy, during labour & delivery and until 6 weeks postpartum. We also provide care for you and your newborn until discharge back to your family physician at 6 weeks postpartum.
Before your first appointment
Please print and complete the New Patient Questionnaire. If you are unable to complete the form prior to your first appointment, then please come 20 minutes early to fill out form in the office.
Please bring your AB Health Care Card and ensure that it is accurate. If you need to update or make changes to your AB Health Care, please visit the Alberta Health Website.
If you are new to Alberta and eligible for AB Health Care, we recommend you apply ASAP to avoid issues with insuring your newborn at the time of delivery. Download the application form and complete using Adobe Reader to ensure you have valid AB Health Care.
